Column Explanation

BREEDER - is the recorded owner of the dam at the time of mating.

OWNER  - is the recorded owner of the dam at the time of birth.
MEMBERSHIP NAME - is the precise wording that will be entered onto the registration form by the Registry after “OWNER” and is identical to that used on the Work Order Form for Membership Name.
FLOCK NAME (abbreviation)  - is a unique name for your flock. If the flock name is Tunis Acres (TA) and the ear tag identification is “333”, then the animal name on the registration will be Tunis Acres TA 333.

SEX - R=ram or E=ewe

EAR TAG ID# - The name that will appear on all your papers in front of the tag or tatoo number.  Most often it should be your last name or farm name. This assigned ID cannot be changed in the registry.

BIRTH TYPE - Of animal being registered/recorded; Sg=single, Tw=twin, Tr=triplet, etc.

BIRTHDATE - Month/day/year of birth of animal being registered or recorded

SIRE REGISTRATION # - Registration/recordation# of sire; upper right corner of sire’s certificate

SIRE FLOCK ID - Flock ID & numbers/letters on ear tag, tattoo of sire

DAM REGISTRATION # - Registration/recordation # of dam; upper right corner of dam’s certificate

Dam ID -Flock ID & numbers/letters on ear tag, tattoo of dam

SIRE or DAM CHRISTENED NAME - can be any available name in the registry that is not already in use; names are available to NTSRI members. There is a one-time $25 fee to record a name.
OWNERSHIP TRANSFER DATE - If animal was sold, list thedate it was sold.

NEW OWNER - If animal was sold, list the name and address of the new owner.

 

VERIFICATION OF MATING - Entries on the form must be from dams or sires owned by the breeder and owner. If this is not the case, complete and submit the VERIFICATION OF MATING form and either email or mail with the order.

Printer Settings

1)Set your printer to "landscape setting"  Set the margins as small as your printer will accept.

2)  Please note - All the information typed in each box will be sent to the ASR OFFICE completely, even though it appears as if some of the info may be left out when it is printed off from YOUR printer.    The NTSRI office will receive all the info if your name has 4 letters or 20 letters!  There is no maximum number of letters or numbers a box will accept.
